The Director of Case Management is responsible and accountable for facilitating integrated clinical services for varied patient populations including new services and chronic care. He/she is accountable for implementing care management programs within Berkshire Medical Center. The Director of Case Management ensures collaboration among health care providers across the Continuum of Care to maximize effective access and efficient utilization of resources. In addition, the Director will ensure that there are systems in place for effective and timely communication with patients, families, providers and insurers. The Director will also facilitate the consolidation of clinical data to ensure timely and accurate measurement of patient outcomes and care processes within Berkshire Health Systems. The Director will appropriately maximize the assignment of inpatient-level of care status to capture and maintain fiscal integrity for Berkshire Medical Center.

Case management experience in acute and managed care and a working knowledge of care management in long-term care and home care.
Minimum of three years’ clinical and administrative experience with progressive responsibility and demonstrated leadership skills.
Knowledge of leadership principles, case management, utilization management, research methods, and outcomes.
Demonstrates effective communication skills, data management, and analysis.
Achieves cooperative and collaborative multidisciplinary relationships with all healthcare disciplines.
Demonstrates performance behaviors which exemplify Berkshire Health Systems Customer Service Standards and Service Excellence philosophy.
Education and Training:
Graduate of an approved school of Nursing.
Current licensure or eligibility as a Registered Professional Nurse in the State of Massachusetts.
Bachelor degree in Nursing or Health related field required, Master’s preferred.
License, Certification & Registration:
RN – Currently licensed as an RN in Massachusetts.
CASEMGMT – CCM (Certified Case Manager), or ACM (Accredited Case Manager) or RN-BC (Registered Nurse, Board Certification in Case Management).
Certification in evidence-based Utilization Management preferred.
